Terms of Service Agreement

1. Introduction Welcome to The Depth Collective PLLC. These Terms of Service (“Agreement”) govern your access to and use of our psychotherapy services. By engaging in therapy with our practitioners, you agree to these terms and acknowledge that the practice operates under the laws and professional guidelines of Illinois.

2. Services Provided The specific purpose of The Depth Collective PLLC is to provide comprehensive clinical psychology services, including but not limited to:

Assessment, Diagnostic and Therapeutic Interventions: Offering individual, relationships, family, and group therapy using evidence-based practices and psychological evaluations to diagnose mental, emotional, and behavioral disorders.

Training, Supervision, Psychoeducational Programs: Providing training and supervision to pre-licensed and licensed psychologists and other mental health professionals.

Consultation Services: Providing expert consultation to individuals and organizations, as well as Advocacy and Outreach: Advocating for mental health policy and participating in community outreach

The aforementioned activities will be carried out in compliance with all applicable laws and ethical standards set forth by the American Psychological Association and relevant state regulatory bodies.

These services are provided within HIPPA regulations and in accordance with Illinois state laws and established professional and ethical standards.

3. Eligibility To receive our services, you must be 18 years of age or older, or have parental/guardian consent if under that age, as per Illinois law.

4. Client Responsibilities

  • Attendance and Cancellation: Comply with our attendance and cancellation policies, including providing 24-hour notice for cancellations.

  • Fees: Payment is due at the time of service unless otherwise arranged, in line with Illinois billing practices for healthcare services.

5. Confidentiality In adherence to the Illinois Mental Health and Developmental Disabilities Confidentiality Act, your information will be kept confidential except in specific circumstances such as imminent risk of harm, abuse, or neglect reporting, or when legally mandated.

6. Therapist Responsibilities

  • Ethical Practice: Therapists will adhere to the ethical guidelines set forth by Illinois law, federal law and professional governing bodies.

  • Limitations: Therapists will inform clients of the limitations and scope of therapy as per Illinois and federal regulations.

7. Informed Consent Before beginning therapy, clients will be provided with detailed informed consent documents, aligning with Illinois law requirements, outlining the nature and course of therapy.

8. Emergency Situations Our practice follows Illinois guidelines for handling mental health emergencies. Clients are encouraged to use emergency services outside of therapy hours. These resources are available on our website ** link to resource page

9. Payment and Insurance We adhere to Illinois' guidelines regarding healthcare billing, including providing necessary documentation for insurance claims and informing clients of their financial responsibilities.

10. Termination of Services Either party may terminate therapy at any time, with termination procedures being in compliance with Illinois state regulations, ensuring a proper referral and continuation of care if necessary.

11. Record Keeping In compliance with Illinois law, clinical records will be maintained and stored securely. Clients have rights to access their records as per the state guidelines.

12. Amendments to Terms These terms may be amended in response to changes in Illinois laws and professional standards. Continued engagement with our services following amendments signifies acceptance of the revised terms.
